get a review assignment of a 1004 and it also has a 1007/216. are you supposed to review that as well? or just the 1004? if all, then how much extra do you guys charge for that?

Depends on your client. Many of our review clients do the review of the 206 and 1007 in house, others want us to review them.

As far as charge, perhaps the same percentage of actually doing a 206 and 1007 as you charge for reviewing the 1004 vs actually doing one?

Agree with Annemieke - Best to take a good look at your scope of work, and the needs of your client/intended users.

Charge?? Now that would be a business decision, nothing governing that at all - I'm willing to bet there would be a lot of discussion of that, here is my guess......
1/4 people on this board doing it for no charge
1/4 of the people charging a lot more to review it
about 1/2 arguing with the ones that do it for free that they are wrong for doing it for free.......
Of course that is simply my opinion, I've been known to be wrong....

50-100 bucks per extra form based on "time served".
