1004d FHA/ead Issue

We've been having issues with ClickForms 1004D certification of completion report. We got the following error msg when submitting to EAD: FHA4200 Effective Date of Appraisal must be provided as a valid date. Effective Date of Original Appraisal field is filled. The Effective Date of Appraisal Update filed is blank since this is certification of completion. We called EAD multiple times, unfortunately the help desk there didn't help much. This seems to be happening only on ClickForms. Any idea? Thanks

I corresponded with the FHA on this issue just this week. We had a report that was failing EAD for the same issue. They told me that this is a known issue with the forms software that you are using, and that if you will obtain the most recent update to your software it should correct the problem.

Another fix is to use SFREP Appraise-it

Tech support is telling me that you're probably not on ClickForms v9.4 Update #3. To check, you can go to Help > About in ClickForms. If you need to upgrade you can download it from our website or you can call our support team at 800-622-8727 and they can get you going in a couple of minutes.
