1007 & 216 -- stand alone or add on?

The assignment is for the mortgage of a vacant SFR being purchased as an investment.

The 1007 and 216 were ordered 3 weeks after the original 1004 was completed.

If the new products are added to the original report, the signature and effective date of the rental survey over-writes the original data resulting in different versions of the report.

If the new products are included in a new stand-alone report, the software requires a base 1004 form that would be blank and possibly misleading.

Just wondering how my peers would deal with the form...
 
I just figured out a work-around. It really doesn't matter what pages are included in the report because the PDF writer allows the appraiser to generate a report that doesn't include all of the forms. Thanks.
