Appraiser unable to obtain condominium docs from HOA or mangement company for conventional lending assignment. Appraisal intermediary consequently advised appraiser to submit report with notation that report will be revised and fully completed when lender provides condo cert. I'm thinking that an EA should be imposed to address any unknown, deleterious affects revealed in the condo cert, but wondering whether CB4 also is needed.

No, CB 4 is for satisfaction of physical condition inspections. Do't make an EA, state the condo information was not made available despite efforts to obtain it and that it will be added as soon as available, and that the comps from same condo prices and marketing times reflect any impact of condo management .
