Does anyone know what form this is? MAS 70/465

I have a processor who left a message asking for it (I don't think she knows what it is either).

The appraisal is for an single family investment property. I first thought she meant a Rent Schedule or OIS but their form numbers are different.

Isn't a 70 a final inspection?

The form 465 is the condo form, I can't find a form 70 reference though.

I called my underwriter and the form 70 is the Freddie number for the Fannie form 1004 (standard old URAR).

Yes, look at the bottom of any URAR! Before Dec, 1993, underwriters would ask for a Addendum 70 when a PUD property was on a URAR. But with the new URAR that became available in 1993, an Addendum 70 was not longer necessary because the new 1004 / 70 has those questions in the PUD section.

You got it, ksappraiser!!

Turns out they wanted a full report and they thought the subject was a condo.

Freddic mac number for full report is 70 but the 465 was throwing me. I prefer the term URAR...

465 in Indy is the interstate number

Thanks all for your responses.
