New Form Addendum ?

I want to generate a template to incorporate into my ACI 1004-05 form report.

It will need to be interactive and it will allow me to check boxes as well as to generate narrative text.

I presume I could do this in the "Condition" narrative addendum but I'm thinking that it might be best as an existing add-on form from the Pak Creator.

Does a blank form exist in ACI that would allow me to manipulate it similar to a word document? I just tried to import word documents into the "Extra Image" and "Extra Document" addendums but it didn't work out.

I am able to import pdf into an extra map page . . .

I Use A Ms Word Doc As A Template By "adding Addendum" Then Inserting That Doc. And Saving As A Template.
It Can Be Edited But Is A Little Slow.
If You Want To Change It Much Each Time , I Would Do It In My Word Processor And Then Insert It.

I'm slower than the software.

Are you referring to a particular "Add Addendum" option, or are you suggesting an ACI template with only the MSWord doc?

Once in the narrative addendum, I found an "Insert Addendum Field" option but I don't think that's what you mean.

I would like to work on the template so the addition/document/etc would be integral into every report of that type...

There is also an "Insert MSWord Doc" option.

If you got a moment please walk me through the process if you don't mind.
