Thanks for your help folks. Appendix D to the 4150.2 is a great refresher! Hopefully I will have all 139 pages read by the time the cows come home. Yikes!

Below is an FHA form that is in my software.
Are we REQUIRED to include it????
If we are: What do we put in areas on the lower left?
Check marks??? dates????
How COMPETE are we required to fill out his form??
All the data it asks for???
Help from youall on the Data entries?? PLEASE???

That looks like an underwriters form. Don't pay any attention to it.

Forms used for FHA are the same used by FNMA (et al) except for completion inspections. Use the CIR (completion inspection report) instead of the 1004D/442 form. Put required repair/inspection items in a comment addendum. The old VC checklist has been retired.
