Question on FHA Re-Inspection Final

Hello;
I recently did an FHA Appraisal where partial utilitis were not on and listing agent and buyers agent refused to turn them on. After a phone call with lender I was told to make appraisal Subject To re-inspection when utilities were on. Lender is now asking me to go back out and due Re-Inspection Final at property for a nice fee I might add.
My question is can I do this on a 1004D form or is there another was to sent these Final Re-Inspection Results?

You will need to use the CIR (Compliance Inspection Report) form 92051.
