Request - Sample of Inspection from Property Data Collector (PDC)

Greetings,

I have yet to complete a 1004P, and not sure if I ever will, but I am looking for a sample of the inspection report that an appraiser receives to complete a 1004P. I really believe that the inspection is an important part of the appraisal process for the appraiser, and I am just trying to determine if the the amount of data that is provided can be used to complete a credible assignment.. Any assistance is greatly appreciated.
