The Form "70"

I was asked to do an appraisal report on the form "70". The underwriter sent an automated fannie may approval to the loan officer with this condition. The underwriter dosen't even know what this form is. It's not a 2065 or a 2070. It's not on the ACI software and I have no clue what it is. Any ideas?

Form 70 is the number used by Freddie Mac for the URAR, Fannie Mae's number is 1004. Look at the very bottom of the URAR, on the left is reads Freddie Mac Form 70 6-93. On the right it reads Fannie Mae Form 1004 6-93.
