I am trying to find a USPAP addendum for reviews in total. I need one that will go thru appraisal port. I talked to alamode tech support, lender and appraisal port and no one can tell what will work. Anyone have a solution to this problem.

Why do you feel the need to include that form?

If you are talking about the SR3 Certification, that should be a pre-printed form, but you can certainly take it from the USPAP and make your own template for your reports (that is what I do).

If you are talking about describing your review SOW, the required review appraisal reporting items (again, refer to SR3), and whatever extraordinary assumptions you and your client have agreed to incorporate into your review assignment, you are not going to find that in a form. You'll have to customize it for your assignment/client.

Good luck!

There are 2 USPAP addenda under the Review subsection of the Contents tab in Total. I don't know if they will pass through Appraisal Port, but if they don't, print it to pdf then insert it in the report and it will go through.

To my understanding, any pdf file will pass through, so the next time save yourself some time in re-typing and print to pdf the n insert the pdf into the report. Quick, easy, simple.

Look under the AIready forms and you will find a USPAP addendum. It is called the USPAP Identification Addendum (USPAP 2014) FNC
