Which Form Do I Need

I need to update DBPR with a change of address. I actually did this on-line when I moved, but my actual license info still shows my old address. I don't need any headaches with the state. :Eyecrazy: I just sent in the Form 2065 with the business info on which I put my own address --- will this suffice?

That was the form that I used, hope it is correct also. All of the information on the DBPR website states that is the form to use. Just send the fax and keep the confirmation sheep for your records as faxes disappear sometimes.

Actually, It would be the DBPR 0080-1 form http://www.myflorida.com/dbpr/dbpr/le_portal/dbpr-0080-1.pdf

I used the 2065 form originally and my address never changed. Called DBPR and they gave me this form to use.
