Where do you find or how do you obtain the installer's Name and Date installed to complete page 2 of the 1004C form? This is different from the date of manufacture and HUD plate etc. Also if you can't find it will your lenders/underwriters accept the appraisal? Thanks for any feedback/help.

In Arizona, the professional licensed installers are required to place a blue tag above the HUD label; that has installation date of several different items. Some times there will be more than one because they might install it on site but not hook up to any utilities until days later. The tag is to be placed above the HUD tag, but I can hear a boss saying to a day laborer "go screw this blue label above the red label". Then the attached photo happens. Check with your local building or zoning department or whatever state or county or city office is responsible for determining if the installation meets their code or not and find out how you can determine that. They might have a similar tag or label. Or one of those office might have the date they issued a Certificate of Occupancy.

Additions or modifications
• Describe any additions or modifications made to the manufactured home since its initial placement on the site. Any subsequent additions or modifications should not take the home out of compliance with the Manufactured Home Construction and Safety Standards (MHCSS). If the appraiser suspects that an addition or modification to the home poses a risk to the structural integrity of the home, the appraiser is to notify the lender.

Installer’s Name Date Installed Model Year
If available, indicate the name of the person or company that installed the manufactured home, the date of installation and the model year of the manufactured home. If information not readily available, the appraiser is to note such.
 
As in Arizona the certificates are required in Florida, and are of such poor quality they rarely survive very long, particularly if placed on the west side. Typically on existing structures I will simply note "Not a new install, no information available"
 
In my market area, the manufactured homes go on record from the "Bill of Sale", separate from the land. For me, I go to the building department and find out what the building permit allows and when the occupancy permit was issued, coupled with the manufactures information in the unit, here we have to disclose the insulation ratings due to our climate along with the other manufactures information.
