1004B Form

I have a lender who wants me to send them a 1004B form (Statement of Limiting Conditions and Appraiser's Certification). Thinking I already have that as part of the report. I have never heard of it. I can't find it in ACI. This is on a FHA which was transfered to them from another lender.

From the lender:
The Statement of Limiting Conditions and Appraiser's Certification (Form 1004B) must be included as an exhibit for appraisals prepared on the Uniform Residential Appraisal Report (Form 1004), the Small Residential Income Property Appraisal Report (Form 1025), the Individual Condominium Unit Appraisal Report (Form 1073), or the Individual Cooperative Interest Appraisal Report (Form 1075). Form 1004B includes ten limiting conditions and nine appraiser's certifications, as well as a supervisory appraiser's certification. To acknowledge that the current version of the Form 1004B was used and to assure the lender that the appraiser is certifying to our current definition of value, the appraiser must insert "06/93" in the blank that references "Freddie Mac Form 439/Fannie Mae Form 1004B (Revised _______________)" in the "Reconciliation" section of the applicable appraisal report form.

I know I can't include it as an exhibit to the report.

Ken....open up an old pre 2005 1004...look at the certifications page at the bottom..
 
Why would they be asking for the old certs on a current appraisal?

Bizarre indeed. Not only do they want the old 1004b certification, but they also ask you to fill in the "(Revised______)" which is in the reconcilliation section of the old URAR.

I don't believe I would send them anything.
