ACI: 1 Text page Addendum ?

Because I am reporting two different values in a single report that includes the 1004 and the 1007 forms, I wish to add an introductory page in which the Scope of Work(s) will be described. It will be the first page after the Table of Contents. I've never added a text page addendum before. Ideas???

Edit: I just typed in Add/Remove....Text Page Addendum, and I now have a blank Text Page Addendum....never thought about looking for it myself rather than asking the AF...
