Desk Review on 2000 Field Review Form

The client wants all desk reviews to be completed on a 2000 field review form. Any problems with this if I report that only a desk review was made. My concern is that everywhere on the form points to the fact that a field review was made.

What the heck is wrong with people?

The pre-printed scope of work page that is not detachable from the report pages should preclude this form for use in "desk" reviews.

That is why I declined the assignment. But people are doing them. I just would not feel comfortable. An updated 2006 form, however, is needed badly.
