Effective Date On Retrospective Appraisal

Hi,
I need help. I am doing an ext. drive by Retro Appraisal and want to verify what date should I put on page 2 of the report: the retro date which is 06/01/2015 or the date that I did Drive By. Thank you for help.
 
The date of value is determined by the client. What date did they want?
 
Oh. I see what the problem is. Don't use a Fannie Form.
 
06/01/2015 is retro date.
 
The retro value date is the effective date, current date is the signature date.
