HUD-92800.5B for Townhouse/PUD

I'm new to FHA since they just raised the loan limits here in Northern CA. Now it's 99% of my business! Here's my question:

Lender asks for HUD-92800.5B be included with the appraisal for a Townhouse/PUD. I found the form in alamode forms. Anyone have any experience with this form and how to complete it?

I've been to the HUD website-- can't find a thing on why this would be required on an existing structure with no construction, repairs, or maintenance needed.

The townhouse is older-- 30 years old, in great shape, no repairs needed, in great neighborhood, maintained very well.

I'm not clear on why this form is needed and how I should proceed. Any help would be much appreciated.

This is not an appraisal form but would be filled out by a HUD/FHA approved inspector for new construction. Appraisers do not do final inspections on new construction.
 
It's not new construction, it's an existing 30 year old townhouse.

So I just leave it blank, attached to my appraisal?? Seems weird...

That form is not applicable to your assignment and is not an appraisal form.
