This question has been going around my associates/appraiser friends & has been an interesting discussion, when you receive a correction request from an in house reviewer (may or may not be a licensed appraiser), request is not from the lender's underwriter/LO, etc, do you change your signature date on your report/form?
 
Correcting errors, I do make a new report and a new report date. The appraisal date stays the same. I do keep the original report. I have had 2 or more reports that were corrections.

In secondary market you need to read the respective rules of the road. Reconsiderations of Value are generally treated differently and those are supposed to come from the underwriter not the loan officer under FHA.
 
i don't for typos, but for everything else new signature dates. technically, a new signature date for any change keeps the changes, dates, easier to spot when you are have 2 appraisal copies in a file, or system.
 
Has the report left the building in any format? You don't want two versions of a report without and date of signature change and a section in the addendum explaining the changes. There has to be integrity in the file copy.
 
Be certain to also change the Appraiser's file number on the Revised Report. Keep copies of BOTH reports in the work file.

Absolutely, positively, no doubt about--new (current) date of signature & report.

Read my mind with that answer. Absolutely - even only one letter or punctuation mark. New file, new number, explanation of change in addendum. It is possible that the date will not change if the modification is so fast that it happens on the same date as the original signature but it will still have a new file number and the addendum explanation.
 
Change teh signature date, and explain in addendum why ( added comments to address reviewer, or made a correction of a typo etc). Adding comments or explanations does not reqire a new file number and is not a few report, it's the same report with a new signature date and corrections or comments added.

If you were to change the value opinion that might be a different matter but even then it does not automatically have to be a new file number or new appraisal. It can be the same report with a new signature date and revised value due to X (explanation).
