Is there a standard draw form that is included in all appraisal software packages? If so, what is the form called? Or does it have a form #?

No there is not. Probably because all draw schedules are different.

No standard form. When I was doing a lot of that work I set up a spreadsheet that would convert a construction budget into % complete on an item-by-item basis. Easy to set up and very handy. Invested a few minutes to set it up and then used it for years.

Several in Alamode: one called Construction Progress Inspection Report, One called Construction Inspection Report, One called Homefocus Construction Loan Inspection Sheet, One called Custom Construction Disbursement Inspection Report (RELS), 5 Stage Disbursement Inspoection Report (RELS), 7 Stage Disbursement Inspection Report (RELS), Manufactured Housing Disbursement Inspection Report (RELS)

Yes, for Alamode if it fits the # draws or per specified client. I have used several and none seam to be ideal for what the client wants. If you are going to get several set up a spreadsheet for fit the needs and drop it in as a PDF or Image to a set of form picture pages and needed addenda.
