Which form to use?

Homeowner requested appraisal for PMI removal. I have done these assignments before but they were always ordered by the lender/AMC. This is the first request directly from homeowner who indicates that his lender told him to find an appraiser. Question...would you do the report using the GP Residential form or a 1004 form? Thanks

I've done a lot of those. I contact the lender to see which form they require.

Also, its not unusual for the owner to contact the appraiser directly in these instances. Collect the fee up front from the owner.
