There is no USPAP rule about which form to use. There are rules about providing an appraisal that is misleading... either intentionally or inadvertently. Tell the Lender (again) that it's an ADU. If they still want it on the 1025.. you can do that... just make sure that you accurately describe the subject, your commentary is clear, and..... that you fee for the additional work is enormous.
