It's called "Appraisal Compliance Addendum" in CF.

You can type them all out (USPAP requirements) or click a box on this pre-printed form.

I'm sure a-la-mode has a similar form, perhaps search your forms for 'USPAP' and it will pop up.

Use a GP(General Purpose) form.
Yes. And in Total (I use that too) it's the "GP Condo" (General Purpose Condo Report)
Do not use the UAD Condo Form as the pre printed Intended Use states "...this appraisal for a mortgage finance transaction"

Agreed, it just that in a la mode, which is the forms program referenced earlier in the thread the form entitled " Appraisal Compliance" has to do with certifying compliance with the Mercury Network vendor management platform and nothing to do with USPAP

I'll figure out how to post the form in the next day or so (couldn't find an example on the www). Searching for comps on a relo this evening to look at tomorrow after job #1. :P
 
Correct Mike. Implemented in '14, still used today (prior services, define 'appraisal report', marketing/exposure). Ala has to have a compatible form somewhere....
