Effective date vs inspection date

When you are completing a retrospective appraisal how do you fill out
the bottom of page 2 of the 1004? It states that the effective date is the date of inspection. In the appraisal I am doing the effective date is 2 years prior to the inspection.

Clickforms General Purpose Non Lender Forms have this same comment at the bottom of page 2. Several of us have recommended they take it off, but so far, only cricket noise as a response.

It would be interesting to hear if other software providers have this commment at the bottom of page 2 or not.
 
Laura... Have you read and thoroughly digested Statement 3 in USPAP?
