I did not read the responses, so may have been answered already. Current VA requirements for this type of issue have changed recently and now are based on local code requirements. So that is where you would go for the answer. I don't hesitate to instruct borrowers to request a waiver from VA if they prefer not to add handrails as in this case which may not look so great, that put's it on VA. However, if local code would require it you still make it a requirement in the report.
Basement steps, conventional financing? Only if I felt there was unusual safety concern. Typically no. VA and FHA would be based on local code requirements which include grandfathering which may eliminate the requirement in older homes.
