HUD retired the form for use by appraisers in April of 2025. They now defer to the 1004d

That form was for new construction stages, not for repairs. Difficult form to understand where to put what for a repair. I hate that form, from only a rew used times. It says the form is for cinstruction stages, duh,

HUD/FHA uses the 92051 for reverse mortgage final assignments that require repairs. I get about one of these a month.
