Haven't seen a "safe room" but I've seen plenty of concealed rooms with access through bookcases, etc. with alternate exits, sometimes between floors. What's becoming more common in Texas is an interior Storm room in the living area. This is due to problems with basements (water table), so the builder has a concrete walled and ceiling room built into the home for a tornado shelter. Essentially an oversized closet, I treat it as part of the living area with a little bit for construction cost.
