The subject is a single detached home with a self-contained suite in the basement. Zoning allows one basement suite. However, the suite is unauthorized and would likely require some work to comply with code.
 
In generic govt agency terms, the planning dept handles zoning issues. The building department handles building issues.

Deficiencies and permit status can sometimes be cured. At a cost.

If you're asking how to complete the form you're using then refer to the guidelines and policies for that form. And that user.
